Transitional HealthCare Solutions, LLC provides Care Transition Coaching and Care Management services for individuals, physician groups, medical organizations, and self-insured employers throughout Northeast Ohio.

The Care Transition program empowers chronically ill individuals and/or their caregivers to take a more informed and active role in their healthcare. However, not everyone can take an active role in their healthcare or even their everyday activities of daily living.  For this reason, Care Management services were added to our service solutions. 

Rebecca Raines, RN & President and founder of Transitional HealthCare Solutions, LLC, possesses Transition Coach^TM credentials, is also an Associate Care Manager and member of the National Association of Professional Geriatric Care Managers.
